Hospice Eligibility
Appropriate identification of hospice- eligible patients increases the likelihood that they and their families will benefit from compassionate, end-of-life hospice care. Healthcare professionals must certify that patients meet Medicare guidelines to be eligible for hospice services with Anthem Hospice Care.
Admission Criteria
- Candidates for hospice care are patients who have been diagnosed with life-limiting illnesses and have a prognosis of six months or less.
- Patient has a declining functional status with a dependence in activities of daily living.
- Patient has alteration in nutritional status
- Patient has an observable and documented decline in overall clinical condition in the past 4-6 months despite aggressive medical treatments
- Patient has had frequent hospitalizations or trips to the ER
- Patient is exhibiting signs of increasing illness such as uncontrolled pain, shortness of breath, and frequent or recurring infections.
Make a Referral
